EĞİTİM HASTANELERİNDE ASİSTAN DOKTORLARIN KLİNİK İŞ YÜKLERİ BAKIMINDAN PERFORMANS ÖLÇÜTLERİNİN KARŞILAŞTIRILMASI
Abstract
Eğitim hastanelerinde klinik iş yükleri ve bu iş yüklerinin performans ölçütlerine nasıl yansıdığı araştırıldı. Dahili ve cerrahi kliniklerdeki asistan doktorlara tanımsal anket çalışması yapılarak karşılaştırma yapıldı. Bu çalışma diğer çalışmalardan farklı olarak hasta memnuniyeti üzerinden değil, performansları değerlendirilen asistan doktor anketlerinden oluşturuldu. Bu çalışmada asistanların iş yükü olarak: Fazla sayıda hasta bakmak, fazla sayıda nöbet tutmak, nöbet izninin olmaması, nöbet ücretinin az olması, yeterli eğitimin verilmemesi, çalışma ortamının uygunsuz olması, eğitimli sağlık personelinin olmaması sonucu çıkarıldı. Dahili ve cerrahi kliniklerde şikayetlerin ve iş yüklerinin ortalama aynı olduğu görüldü. Ancak cerrahi kliniklerin girişimsel işlemlerden dolayı performans puanının daha fazla olduğu ve bunun da kazançlarına yansıdığı tespit edilmiştir. Performansı düşüren iş yüklerinin aynı zamanda performans ölçütlerini de olumsuz yönde etkilediği görüldü. Ayrıca hangi düzenlemelerin performans artırıcı etkileri olduğu üzerinde duruldu
Abstract
It is analyzed that clinical workloads in Education Hospitals and how this
workloads effect to performance measures. The comparison was done by making
questionnaire study to assistant doctors who works at internal and surgical clinics.
This study is composed with questionnaire studies of the assistant doctor’s performance
evaluation differently from patients satisfaction. In this study workload of the
assistants were mainly described as; examining too many patients, being on night
duty more times, no permission for rest after night duty, low amount of surcharge
rates for night duty, low quality of the medical education, unsuitable work environments,
not well educated allied health personnel in policlinics. It is determined that
the complaints and workloads for internal and surgical clinics were the same. But
it is observed that the surgical clinics have more performance points because of the
invasive procedures and it was reflected to their earnings.
It is observed that these workloads which decrease the performance were
also effected negatively to performance measures. Also overemphasized which are
productive arrangements
