Hemogram Parametrelerinin Disfonksiyonel Uterin Kanama ve Hormon Düzeyleri ile İlişkisi

Abstract

Giriş: 
Disfonksiyonel uterin kanama organik sebep olmaksızın meydana gelen adet
düzensizliği olarak tanımlanmaktadır. Literatürde birçok çalışmada hemogram
parametrelerinin çeşitli hastalıklardaki değişimi irdelenmiştir.  Bu çalışmada hemogram parametrelerinin
disfonksiyonel uterin kanama ve hormon düzeyleri ile olan ilişkisini incelemeyi
amaçladık.

Gereç
ve Yöntem:
Çalışmamıza adet
düzensizliği olan ve  
disfonksiyonel uterin kanama
teşhisi konulan
42 kadın çalışma grubunu ve düzenli adet gören 45 kadın da
kontrol grubunu oluşturmak üzere toplam 87 kadın dahil edildi.
Disfonksiyonel uterin kanama
teşhisi olan kadınların
temel demografik özellikleri ve laboratuvar testlerinin
sonuçları hastanemiz jinekoloji polikliniğine gelen kadınların kayıtlarından
retrospektif olarak elde edildi.
 Hemogram ve hormon parametreleri bakılmış olan
hastaların sonuçları oluşturulan formlara aktarıldı.

Bulgular: çalışma ve kontrol grubundaki olgular
yaş ve vücut kitle indeksi (VKİ) açısından eşleştirildi, her iki parametre de
gruplar arasında benzerdi (p> 0.05). Disfonksiyonel uterin kanama olan kadınların
ve kontrol grubunun RBC ve PDW değerleri arasında istatistiksel olarak anlamlı
bir farklılık vardı (4,520±0,3384 vs 4,741±0,4126 p=0,008), (15,851±0,5911 vs
16,262±1,474 p=0,029). Grupların hormon parametre değerleri ( FSH, LH, TSH, PRL)
ve hemogram parametre değerleri (HB, HCT, PLT, MCV, MPV, RDW, MCH, MCHC, PCT)
açısından istatistiksel bir fark izlenmedi. FSH ile MCV (r=0,227 p=0,035) ve MCH  (r=0,292 p=0,006) arasında istatistiksel
olarak anlamlı bir pozitif korelasyon ve FSH ile RDW (r=-0,225 p= 0,037)
arasında ise istatistiksel olarak anlamlı bir negatif korelasyon saptandı.







Sonuç ve Tartışma: Çalışmamızda PDW ve RBC disfonksiyonel uterin kanaması olan hastalarda
anlamlı olarak yüksek saptanmıştır. Hormonal parametrelerden FSH ile MCV, MCHC,
MCH ve RDW arasında anlamlı bir ilişki saptanmıştır. Bu parametreler  disfonksiyonel uterin kanaması olan kadınların
tanısında  yardımcı bir belirteç olarak
kullanılabilmesi muhtemeldir.
