2000’li YILLARDAN İTİBAREN TÜRKİYE’DE SAĞLIK POLİTİKALARINDAKİ DEĞİŞİMİN ÇOKLU AKIŞ MODELİ İLE ANALİZİ

Abstract

1970’li yıllarda yaşanan ekonomik kriz ile birlikte gündeme gelen neo liberal söylemler küresel düzeyde özelleştirme uygulamalarını da ortaya çıkarmış ve yaygınlaştırmıştır. Türkiye’ye de uzanan bu uygulamaların yansımaları sağlık sektörünü de etkilemiş ve dönüşüm sürecine sokmuştur. Dönüşüm, reform transferi yoluyla başta Uluslararası Para Fonu ve Dünya Bankası olmak üzere Dünya Sağlık Örgütü gibi uluslararası örgütler tarafından gündeme getirilmiştir. Bu doğrultuda Türkiye’de 1990’lı yıllarda reformlar yapılmaya çalışılmış fakat istikrarsız siyasi ortam nedeniyle etkili politikalar hayata geçirilememiştir. 2000’li yıllara gelindiğinde ise yeni kurulan ve tek başına iktidara gelen Adalet ve Kalkınma Partisi’nin önerileriile gerek kalkınma planları gerekse hükümet programlarında özelleştirme ve piyasa mantığı yeniden gündeme getirilmiştir. Böylece seçimlerin ardından 2003 yılında Sağlıkta Dönüşüm Programı hazırlanarak yürürlüğe konulmuştur. Bu çalışmada, bu program çerçevesinde yürürlüğe konulan politikaların nasıl şekillendirildiği, hangi aktörlerin sürece katkısının ön plana çıktığı ve bu politikaların sonuçları detaylı bir şekilde açıklanmaya çalışılacaktır.
Literatür taraması yoluyla elde edilen bulgular Kingdon’un 1984 yılında geliştirdiği Çoklu Akış Modeli ile analiz edilmiştir. Bu çalışmada2000’li yıllarla birlikte gündeme gelen sağlıkta dönüşüm politikaları, “Çoklu Akış Modeli”kullanılarak; problemlerin akışı, politikaların akışı ve siyaset akışı şeklinde ele alınıp incelenmiştir. Çalışma sonucunda dönüşüm sürecine giren sağlık sektörünün hem hizmet sunucular hem de bu hizmetten fayda sağlayanlar açısından birtakım sorunlara neden olduğu görülmüştür.

MULTI-FLOW FROM THE 2000 MODEL YEAR CHANGE IN HEALTH POLICY IN TURKEY AND ANALYSIS

Abstract

Neo-liberal discourses, whichcametotheagendawiththeeconomiccrisis in the 1970s, alsorevealedandspreadprivatizationpractices at the global level. thehealthsector has alsoaffectedthereflections of thesepracticesalsoextendtoTurkeyand has put theconversionprocess. Transformation, has beenbroughttotheagendabyinternationalorganizationssuch as the International MonetaryFundandthe World Bank andthe World HealthOrganizationthrough reform transfer. Inthisdirectionhaveattemptedreforms in Turkey in theyear 1990 but was not implementedeffectivepoliciesduetotheunstablepoliticalenvironment. Inthe 2000s, withthesuggestions of thenewlyestablishedJusticeand Development Party, whichcametopoweralone, thelogic of privatizationand market wasbroughtupagain in developmentplansandgovernmentprograms. Thus, aftertheelections, theHealthTransformation Program waspreparedand put intoeffect in 2003. Inthisstudy, wewilltrytoexplain in detail how thepoliciesenactedwithintheframework of this program areshaped, whichactorscontributetotheprocessandtheresults of thesepolicies.
ThefindingsobtainedthroughtheliteraturereviewwereanalyzedwiththeMultipleFlow Model developedbyKingdon in 1984. Inthisstudy, healthtransformationpolicies, whichcametotheagenda in the 2000s, usingthe "MultipleFlow Model"; theflow of problems, theflow of policiesandtheflow of politicshavebeenexaminedandexamined. As a result of thestudy, it wasseenthatthehealthsector, whichenteredthetransformationprocess, causedsomeproblems in terms of both service providersandbeneficiaries.
