EPİDERMOLİZİS BÜLLOZA TANILI ÇOCUĞUN ROPER LOGAN TİERNEY HEMŞİRELİK MODELİNE DAYALI HEMŞİRELİK BAKIMI: OLGU SUNUMU

Öz

Epidermolizis Bülloza (EB); sistemik, kalıtsal ve kronik otoimmün bir hastalıktır. Hastalık her iki cinsiyeti de etkileyebilen, ırk ve etnik gruba göre farklılık gözetmeden; el, ayak, bacak, özafagus, oral mukoza gibi vücudun farklı bölgelerinde içi su dolu kabarcıklar ile seyreder.. Bir milyon canlı doğumda 19 bebekte görülen bir hastalıktır. EB’ye sahip çocukların klinik tablosu çok ağırdır; yaşamsal organların mukozasında artan büllere bağlı olarak morbidite ve mortalite oranı yüksektir. Hastalık vücudun tüm organlarını sistemik olarak etkileyebilmektedir. Kronik hastalıklarda çocuğun ve aile üyelerinin hastalığa uyumunu sağlama, bakımını sürdürme ve yaşam kalitesini arttırma amacıyla pediatri hemşirelerine önemli sorumluluklar düşmektedir. Bu aşamada bakımın etkinliğini arttırmak, sistematize etmek, rehber oluşturmak ve profesyonelliğini artırmak amacıyla hemşirelik kuram ve modelleri kullanılmaktadır. Literatürde kronik hastalığı olan çocukların bakımında Roper Logan Tierney’in Hemşirelik Modeli Yaşam Aktivitelerine Dayalı Hemşirelik Modeli’nin kullanıldığı sınırlı sayıda çalışma bulunmaktadır. Bu olgu sunumunda nadir görülen bir hastalık olan EB tanılı bir çocuğun hemşirelik bakımında Roper Logan Tierney’in Hemşirelik Modeli Yaşam Aktivitelerine Dayalı Hemşirelik Modeli referans alınarak bakım sunulmuştur. Olgunun analizinde, bağımlılık durumunun değerlendirildiği, 12 yaşam aktivite modeline göre 6 spesifik aktivite (Hastanın ve Çevresinin Güvenliğini Sağlama Sürdürme iletişim, beslenme, boşaltım, bireysel temizlik ve giyinme, ölüm) ele alınarak bakım planı oluşturulmuştur.

NURSING CARE FOR CHILD DIAGNOSED EPIDERMOLYSIS BULLOSA BASED ON THE ROPER, LOGAN, TIERNEY NURSING MODEL: A CASE REPORT

Öz

Epidermolysis Bullosa (EB); It is a systemic, inherited and chronic autoimmune disease. The disease can affect both genders, regardless of race and ethnic group; It progresses with blisters-filled with water in different parts of the body such as hands, feet, legs, esophagus, and oral mucosa. It is a disease that seen in 19 infants per one million live births. The clinical picture of children with EB is very severe; Morbidity and mortality rates are high due to increased bullae in the mucosa of vital organs. The disease can affect all organs of the body systemically. In chronic diseases, pediatric nurses have important responsibilities in order to ensure the adaptation of the child and family members to the disease, to maintain their care and to increase their quality of life. Take care at this stage, nursing theories and models are used in order to increase its effectiveness , systematize, create a guide and increase it professionalism. There are a lımıted number of studıes ın the literatüre, in which Roper Logan Tierney's Nursing Model Based on Life Activities Nursing Model is used in the care of children with chronic diseases. In this case report, the nursing care of a child diagnosed with EB, which is a rare case disease, care is provided with reference to Roper Logan Tierney's Nursing Model Based on Life Activities Nursing Model. In the analysis of the case, a care plan was created by considering 6 specific activities (Ensuring the Safety of the Patient and His Environment, maintaining communication, nutrition, evacuation, individual cleaning and dressing, death) according to 12 life activity models, in which the addiction status was evaluated.
